Guidelines for Submitting a Medical Claim
1. Complete the claim Form (Forms can be obtained from our offices, your health care provider, or here online.)
- Fill out all required personal information in the designated sections.
- Affix your signature in the appropriate area.
- Ensure the attending doctor completes and signs the form.
2. Attach Supporting Documents
- Securely attach original receipts for all medical expenses to the claim form.
3. Submit Your Claim
- Bring the completed claim form and all supporting documents to our office for processing. There is a 90-day window for the submission of a claim.
